SECTION TWO
1. PLEASE BE SPECIFIC, e.g., indicate membership, role played in carrying out mission of the organization, time spent each semester. (Use a separate sheet, if necessary.)
2. List your activities outside your high school (or college, if applicable), including civic, community, charitable, religious, and other activities, in which you have participated. Please be specific, and use a separate sheet, if necessary.

SECTION THREE
I. ESSAY
Please prepare an essay (no longer than 750 words) explaining: (1) your interest in pursuing a career in business or the securities industry; (2) the steps you are taking to prepare yourself for this career.
II. TRANSCRIPT
Please include your official school transcript (with raised seal or other similar mark of authentication) with this application.
III. REFERENCES
Using the recommendation forms provided by the FSDA Scholarship Committee (included with your application package), please submit two letters of references - one academic (by a faculty member or dean at the school currently or most recently attended by you) and one non-academic.
IV. BIOGRAPHICAL SKETCH
Submit a one paragraph (300 words or less) biographical sketch.
APPLICATION DEADLINE: All applications, plus supporting documentation, must be received by the FSDA Scholarship Committee no later than May 15, 2007 for the fall semester of 2007.
WHERE TO SEND YOUR APPLICATION: Your Application for Scholarship and supporting materials must be submitted in a sealed envelope addressed and mailed to: FSDA SCHOLARSHIP COMMITTEE, P.O. Box 12092, Tallahassee, Florida 32317-2092.
